Community Staff Community Nursing Community Nurses are employed by the Primary Care Trust and work alongside the Medical Centre staff based at the surgery. District Nurses District Nurses play a crucial role in the Primary Health Care team. They visit people in their own homes or in residential care homes, providing care for patients and supporting family members. As well as providing direct patient care, District Nurses also have a teaching role, working with patients to enable them to care for themselves or with family members teaching them how to give care to their relatives. District Nurses play a vital role in keeping hospital admissions and readmissions to a minimum and ensuring that patients can return to their own homes as soon as possible. District Nurses assess the health care needs of patients and families, monitor the quality of care they are receiving and are professionally accountable for delivery of care. They work with patients of any age, but often many of them are elderly, while others may have been recently discharged from hospital, be terminally ill or have physical disabilities. District Nurses visit patients as required, offering help, advice and support. They liaise with other groups, such as the Social Services, voluntary agencies and other NHS organisations and help to provide and co-ordinate a wide range of care services. Early detection of ill-health, monitoring of growth and development and Health Promotion are their responsibility, with particular reference to mothers, babies and the under 5’s. The Health Visitor liaises closely with the GPs, other members of the nursing team, children’s centres and professions allied to medicine such as Speech Therapists, Special Educational Needs Co-ordinators and Social Workers. The Health Visitor has knowledge of the facilities and resources available to families and children in the area. She will see clients at home or in the surgery and runs weekly clinics. In addition to running Ante-Natal clinics, they work with the Health Visitor in helping to prepare couples for the birth and care of their baby. The same Midwives will deliver mothers who wish to have their babies at home or in the Maternity Unit at Chipping Norton Hospital and continue to care for them whilst in that hospital and at home afterwards.
